25 | On 6/12/26, Licensing Program Analyst (LPA) Felisa Shirley conducted an unannounced required annual visit. LPA met with the Administrator, Bienvenido ‘Bien’ Cadungog and explained the purpose of today’s visit. The purpose of today’s visit was to conduct the required annual inspection, using the new Care Tool. The facility is licensed approved for age range 60 and over; Ambulatory and Non-Ambulatory, walk in closet shall not be used for sleeping or living quarters. Approved hospice waiver for one client only. The census is currently 5.
Morningside Terrace is a one-story structure, located in a residential neighborhood. LPA Shirley reviewed (5) resident files and (3) staff files. The facility consists of (6) resident bedrooms, (1) staff room, (3) bathrooms, living room, kitchen, dining area, shaded patio, and the laundry room enclosed in the kitchen. Facility maintains all required posting throughout the facility.
LPA Shirley and Bien walked throughout the facility and all common areas such as the living room, dining room, and outdoor patio were all observed. The medication is stored in a cabinet in the kitchen, and sharps are stored in a kitchen drawer, locked and inaccessible to clients in care. Bathrooms were found to be within Title 22 regulations and were clean and operational. The water delivered at 111 F.
LPA Shirley observed that there is ample seating and space for all clients. All walkways were clean, and clear of obstructions and hazards. All areas have ample lighting. There are (2) charged fire extinguishers last serviced 10/2025. Exterior areas are clean and clear of obstructions and hazards and there are no bodies of water present. The first aid kit was fully stocked and there was a first aid manual available.
No deficiencies were cited during this inspection visit.
An exit interview was conducted, and a copy of this report was provided to the Administrator, Bienvenido “Bien” Cadungog.
