25 | On June 20, 2026, Licensing Program Analyst (LPA) Ernand Dabuet conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with KRISTINE SIMONIAN administrator. LPA explained the purpose of today’s visit. The facility is licensed to operate for six (6) non-ambulatory residents ages 60 and over with an approved hospice waiver for three (3) residents. Currently there is only one (1) hospice resident in care.
The facility is a single-story structure located in a residential neighborhood. It consists of the following: five (5) resident bedrooms, two (2) bathrooms, a living area, a dining area, a kitchen, a garage, a laundry room, a staff office, side patio, and (2) outside covered patio area.
LPA toured the physical plant. There were no bodies of water or obstructions on the premises. All rooms were inspected. Beds and bedding supplies were in good condition, adequate lighting was provided, and storage for the resident's personal belongings was observed. Bed linens, comforters, and bath towels were adequately stocked at the time of the visit. Bathrooms were found to be within Title 22 regulations and were clean and operational. A water temperature of 105.0 degrees F. A comfortable temperature of 71 degrees F. was maintained in the facility.
LPA observed the facility to be sanitary and appropriately furnished during the visit. Storage areas for personal hygiene, cleaning supplies, toxins, and sharps objects were stored and not accessible to residents. The kitchen was inspected and there are sufficient perishable and non-perishable food available and maintained adequately. The fire extinguishers were charged, and smoke detectors and carbon monoxide were operable. The facility conducted a Fire Drill on 04/01/26 through 04/03/26.
