25 | Licensing Program Analyst (LPA) Tao conducted an unannounced annual visit at the facility using the CARE inspection tool. Upon arrival, LPA met with Assistant Administrator Indu Amarasinghe. The reason for the visit was explained.
The facility is licensed to serve forty-eight (48) non ambulatory residents who are ages 60 and above. Annual fees are current. The facility is a three-story building located in skid row area in downtown LA. On the first floor, it consisted of an activity or group room for residents, dining area, one (1) resident bathroom, three (3) staff rooms, one (1) staff break room, one (1) staff bathroom, laundry room with multiple washers and dryers, an industrial kitchen and a medication room. On the second floor, it consisted of twelve (12) shared resident bedrooms, residents’ group room, nurse station, one (1) big bathroom with multiple stalls and showers, cleaning supply room, four (4) linen closets, and emergency food supply room. On the third floor, it consisted of twelve (12) shared resident bedrooms, one (1) residents’ group room, nurse station, one (1) big residents’ bathroom with multiple stalls and showers, office, cleaning supply room, and four (4) linen closets.

Infection Control: Sufficient PPE supplies and an Infection Control Plan maintained at the facility were observed and in compliance.
