Investigation Finding: Unsubstantiated
On 06/02/22, LPA M. Garcia-Centeno conducted interviews with reporting party (RP), staff (ADM) and obtained facility records. On 06/06/26, LPA K. Nguyen attempted to contact Pacifica Senior Living Encinitas and found the number is no longer in service. On 06/06/26, LPA K. Nguyen attempted to contact Pacifica/ Atria Encinitas South Senior Living now operating at the same address with no answers and no messages can be left. LPA conducted a Google search for the address of the complaint facility, and the address matches that of Atria Encinitas Senior Living; therefore, LPA K. Nguyen contacted the facility and found that it had no association with Pacifica Senior Living Encinitas following a change in ownership. Review of facility records confirmed that there was a change of ownership and the license for Pacifica Senior Living Encinitas closed on 01/20/23. Due to lack of obtainable information, the allegation cannot be substantiated. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did occur, therefore the allegation that there was insufficient staff to meet residents’ needs was found to be unsubstantiated.
Allegation: Staff did not provide shower assistance to residents
Investigation Finding: Unsubstantiated
On 06/02/22, LPA M. Garcia-Centeno conducted interviews with reporting party (RP), staff (ADM) and obtained facility records. Review of the annual inspection conducted by LPA R Serrano at the facility showed that on 03/06/22 LPA toured the facility inside and outside as well as interviewed residents in care. No deficiencies were cited during the annual inspection. On 06/06/26, LPA K Nguyen attempted to contact Pacifica Senior Living Encinitas with the phone number provided with no answers and no messages could be recorded. LPA attempted to contact RP and former staff (ADM) with the numbers provided and found that the phone services were no longer valid. Review of facility records confirmed there was a change of ownership and the facility license for Pacifica Senior Living was closed on 01/20/23. Due to lack of obtainable information, the allegation could not be substantiated. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did occur, therefore the allegation that staff did not provide shower assistance to residents was found to be unsubstantiated.
